Social Worker

 

            

             The Support Center provides social work services to the participants at the center, their families, as well as individuals in the community. These services include but are not limited to: inquires, intake procedures, community referrals, assistance with Medicaid applications, and individual and caregiver support.

 

             The Social Worker works closely with the nursing department to assist families, caregivers, and other affiliated parties for a smooth transition thru the process. The Social Worker attends all care plan meetings, helps to develop and update the individualized care plan for each participant.

             The Social Worker also monitors the participant’s adjustment to the center, as well as noting any changes in behavior throughout their stay. The Social Worker is available for any questions or concerns regarding the center, available resources, and care planning for you and your loved one.
